About The Position

This role involves providing direct care and assistance to residents, ensuring their safety, well-being, and comfort. The Certified Nursing Assistant will monitor residents' physical, mental, and emotional conditions, document observations and care provided, and collaborate with the charge nurse to address any changes or concerns. Responsibilities include assisting with daily living activities, maintaining a safe and clean environment, and encouraging resident participation in activities and therapies.

Requirements

Must have completed a nurse aide program and obtained certification.

Responsibilities

Ensure residents receive assistance with care, to whatever level required and/or ordered.
Complete and document resident tasks within EMR with accuracy.
Obtain and document vital signs and weights within EMR.
Monitor and document resident behaviors, notifying the charge nurse as needed.
Prioritize resident safety by monitoring and preventing falls, ensuring safety devices are in place and functioning properly, checking special alarms and call bells, ensuring correct bed positioning, and completing transfers as required.
Use lifts with proper equipment and staff assistance.
Make rounds at least every two hours to monitor assigned residents.
Answer call lights promptly, providing courtesy and care as needed.
Complete documentation as needed.
Encourage residents to participate in activities, therapies, and socialization.
Monitor and observe residents’ physical, mental, and emotional conditions, reporting changes immediately to the charge nurse.
Complete documentation regarding observations to ensure compliance and trigger alerts.
Comply with facility policy regarding toileting and incontinence.
Follow bowel and bladder protocols for assigned residents.
Accurately monitor for bowel issues, reporting concerns to the charge nurse.
Monitor for lack of urine output.
Accurately maintain Intake & Output.
Ensure proper foley catheter placement and monitoring.
Monitor for proper diet and note any diet issues.
Assist with feeding residents as needed.
Ensure proper hydration, with fresh water available and within reach.
Monitor skin and alert the charge nurse to concerns.
Turn residents as ordered to prevent skin breakdown.
Complete assignments as ordered, performing cleaning, stocking, and keeping resident areas orderly and neat.
Participate in Emergency Preparedness drills as required.
May be assigned to any unit as staffing needs change with short notice.
May be required to assist the agency or state government generally in the event of an emergency declaration by the Governor.
