About The Position

Moments Hospice is hiring a Certified Nursing Assistant to provide personal care and support to patients during their final days. The CNA works under the direction of a Case Manager RN and reviews patient care plans while assisting with activities of daily living, mobility, nutrition, and other patient needs. The role is described as on the road and includes a company car benefit option, with fuel and insurance covered; the vehicle may also be used for personal use at minimal cost. A company cell phone, supportive team environment, reasonable caseloads, on-call support, and structured training are also described.

Requirements

• Certified Nursing Assistant (CNA) license.
• At least 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
• Excellent customer service skills.
• Strong written and verbal communication and active listening skills.
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license.
• Valid auto insurance.

Nice To Haves

• Current CPR certification.

Responsibilities

• Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
