About The Position

The primary purpose of this job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves a variety of essential duties focused on resident care, safety, and comfort, as well as accurate record-keeping and communication within the healthcare team.

Requirements

• Minimum of a 10th-grade education.
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
• Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
• Regularly required to sit, stand, walk, talk, read, or hear.
• Frequent use of all office-related equipment to include copier/scanner/fax, telephone, and calculator.
• May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

• Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
• Identify residents before administering treatments or serving meals using wristbands or photo cards.
• Record all entries on flow sheets, notes, and charts in an informative and descriptive manner, using only authorized abbreviations.
• Report all changes in resident condition, accidents, and incidents to the Nurse Supervisor/Charge Nurse promptly.
• Maintain resident confidentiality and report any suspected or known violations of protected health information or unauthorized attempts to access the facility’s information system.
• Perform only trained nursing care procedures and ensure resident rooms are prepared for new admissions.
• Greet, escort, and introduce new residents, making them comfortable.
• Inventory, mark, and store resident's personal possessions and clothing.
• Assist residents with packing for transfers or discharge and transport them as needed.
• Create and maintain a warm, positive, and calm environment.
• Participate in regular meetings with nursing personnel to identify and correct problem areas.
• Report all resident complaints and grievances.
• Participate in and receive nursing reports.
• Follow established policies concerning exposure to blood/body fluids and report occupational exposures.
• Make beds, put extra covers on beds, and measure/record resident vital signs (TPRs) and weight/height.
• Ensure residents unable to call for help are checked frequently and answer resident calls promptly.
• Check residents routinely to ensure personal care needs are met.
• Assist residents with identifying food arrangements and record food/fluid intake, reporting changes in eating habits.
• Keep resident water pitchers clean, filled, and within reach.
• Perform after-meal care.
• Follow work assignments and schedules, cooperating with inter-departmental personnel.
• Notify the facility when late or absent.
• Assist residents with daily functions such as dental/mouth care, bathing, dressing, and undressing.
• Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
• Assist residents in preparing for medical tests and examinations.
• Assist with lifting, turning, moving, positioning, and transporting residents.
• Perform restorative and rehabilitative procedures as instructed.
• Observe and report skin breakdowns and pressure areas to prevent decubitus ulcers.
• Report injuries of unknown source, including skin tears.
• Observe disoriented and comatose residents and record/report data.
• Perform special treatments as instructed.
• Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
• Use only trained equipment and report defective equipment.
• Maintain confidentiality of all resident care information and report unauthorized disclosures.
• Report all allegations of resident abuse and/or misappropriation of resident property.
• Honor resident's refusal of treatment requests and report such requests to the supervisor.
