About The Position

Clinical participant in the Person Centered Care operational team. Ensures activities of daily living are delivered according to the wishes of each resident. Actively communicates in the care plan process via effective communication, implementation and evaluation of the care received by each resident.

Requirements

Certification as a nurse’s aide in the state of Missouri.
Ability to safely lift and transfer residents weighing an average of 150 pounds with or without assistance, on an occasional basis.
Ability to push residents in wheelchairs up to 250 pounds on a frequent basis approximately 100 feet.

Responsibilities

Understand the importance of customer service and be willing to learn resident/relative satisfaction techniques as a participant in the Person Centered Care program. Accepts the role of a CNA in the ability of each resident to reach his/her highest level of physical and psychosocial well-being. Every resident to be treated in a dignified manner and as an individual.
Perform basic nursing skills safely and efficiently as outlined in facility educational program and nursing policies and procedures. Thoroughly understands policies concerning: prohibition of resident abuse; fall prevention, incontinence care, nutritional program; elopement protocols, skin care and pain management.
Document in the nursing assistant notes the care and treatment provided to the resident and the resident's response or lack of response to care provided as required by the facility.
Complete food percentage, intake and output and bowel and bladder records.
Participate in fine dining experience for residents by ensuring each person is ready for meals, i.e. face and hands washed, teeth brushed, hair brushed, etc. Assist with transport to dining room as necessary. Assist dietary server with beverages, feed dependent residents and assist with transport of resident as needed from dining room.
Provide incontinent care as per facility protocol, with emphasis on toileting per each resident’s normal schedule and maintaining B&B continence.
Perform restorative nursing as outlined in each resident’s plan of care. Includes ambulation, range of motion, etc.
Make sure all residents are offered nourishment during the day as requested and an evening snack if appropriate. Document intake of snacks if such is careplanned. Ensure food and supplies in satellite kitchen are consumed by residents only.
Perform tub baths, showers or bed baths as per the careplan and resident choice. Understand philosophy of Bathing Without a Battle and goal of bathing is for resident to remain free from body odor and skin breakdown. Report any skin issues noted during bathing to charge nurse.
Turn and reposition all dependent bed and chair residents as required.
Empty and record all urinary drainage units.
Assist in dressing and undressing residents as necessary and assist residents in hygiene related activities including oral care and denture care. Allow residents to choose clothing to be worn on a daily basis.
Understand every member of Person Centered Care team is responsible for assistance with activities. Any spare time of the CNA is spent with a resident, whether it be talking, reading, etc.
Assist residents in preparing for medical tests, social activities, appointments, etc. and assist in transporting residents to and from various location.
Provide post-mortem care as instructed.
Report incidents, including near misses and changes in residents’ condition to charge nurse or Dir. of Nurses immediately.
Identify safety hazards and emergency situations, report them, and initiate corrective action. Understand it is the duty of each and every employee to know how to safely operate equipment.
Participate in resident care conferences, assist in completion of Minimum Data Set (MDS) and deliver nursing care as per the careplan. Remind new employees of the important of careplans in the Person Centered Care operational model.
Follow facility procedures for and document admission, discharge, and transfer of residents.
Participate in facility education programs and assume responsibility for achieving the mandatory 12 hours of in-services per year.
Observe facility infection control procedures, disaster preparedness procedures and Corporate compliance rules.
Observe Resident's Rights policies, HIPAA rules and prohibition of abuse/neglect. Understand every employee of a skilled nursing facility is a mandated reporter.
