About The Position

This role involves providing direct care to residents during the second shift, from 2:30 PM to 11:00 PM. Responsibilities include understanding resident status from the outgoing shift, ensuring residents are clean and properly dressed, assisting with bathing, grooming, feeding, and maintaining a clean living environment. The position also requires monitoring residents for any unusual incidents, checking vital signs, and ensuring all documentation is completed accurately before the end of the shift.

Requirements

Certified Nursing Assistant by the State of South Carolina.
Active and in good standing with the State of South Carolina.

Responsibilities

Check group assignment at the main desk.
Make rounds with the outgoing shift to learn about residents' status.
Ensure residents are dry prior to accepting the assignment.
Report any problems to the nurse.
Check shower list and ensure residents are bathed and dressed properly before taking them to the dining room.
Feed residents on the hall and then go to the designated dining room or hall.
Ensure all residents have their scheduled bath or shower, and notify the nurse if a resident refuses.
Ensure residents are properly groomed, clothes are matched and appropriate for the weather, and nails are clean; shave as needed.
Report any resident refusal to eat to the nurse.
Give 3 PM snack and record it.
Give ice if there is no resident assistant.
Report any unusual incidents such as falls, skin tears, bruises, different behaviors, or anything not normal for a resident.
Keep rooms clean.
Turn and reposition residents as scheduled.
Complete PCR books before the end of the shift.
Ensure shower and snack lists are signed.
Keep all closets clean.
Clean wheelchairs and geri-chairs as needed.
Assist residents to and from activities and the dining room.
Keep residents' personal belongings labeled and bagged.
Check vital signs weekly and PRN.
Complete meal records.
Take dirty barrels out and bring empty barrels for the next shift.
Follow the chain of command.
Perform mouth care before putting a resident to bed.
Remove and soak dentures.
Ensure all residents are in bed clothes prior to putting them to bed.
Be knowledgeable about proper restraint and treatments for residents.
