About The Position

The Certified Nursing Assistant (CNA) delivers personal care and minimal homemaker services that promote a safe and clean environment for hospice patients in their homes, assisted living facilities, or long-term care facilities. Services are provided in accordance with Hosparus Health policies and the CNA plan of care (POC). The CNA also participates in developing policies and procedures that guide the provision of hospice care.

Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m., with on-call requirements. Service area: Louisville and surrounding communities.

Requirements

• CNA certification in the state where hired to work, or an active LPN or RN license.
• Valid driver's license.
• Auto insurance coverage.

Nice To Haves

• High school diploma.
• One year of home health aide or nursing assistant experience.
• Experience using technology-based tools, including electronic medical records.

Responsibilities

• Deliver personal care to hospice patients in home, assisted living, or long-term care settings.
• Provide minimal homemaker services that promote a safe and clean patient environment.
• Provide services according to Hosparus Health policies and the CNA plan of care.
• Participate in developing policies and procedures that guide hospice care.
