About The Position

Rocky Mountain Care is seeking compassionate and dependable Certified Nursing Assistants (CNAs) to join their growing home health services team in Rawlins and Carbon County, Wyoming. This community-based role offers the opportunity to build one-on-one relationships with clients in their homes, helping them remain comfortable, safe, and independent. CNAs in this role will travel throughout Carbon County to provide care. The position emphasizes providing personal, compassionate care and making a meaningful difference in clients' lives.

Requirements

• High school diploma or equivalent
• Current CPR/BLS certification
• Must have no disciplinary action in effect against certification.

Nice To Haves

• Certified Nursing Assistant in good standing with the state
• At least one year of experience as a certified nursing assistant in long-term care, home care and or hospice

Responsibilities

• Providing compassionate personal care in clients' homes
• Assisting with bathing, grooming, dressing, toileting, and other activities of daily living
• Assisting with mobility, transfers, and positioning
• Helping with meals and hydration as appropriate
• Taking and documenting vital signs and other assigned observations
• Observing clients for changes in condition and communicating concerns to the appropriate member of the care team
• Providing companionship and emotional support
• Supporting clients in maintaining a safe and comfortable home environment
• Following individualized care plans and established care procedures
• Communicating professionally with clients, families, and members of the healthcare team
• Maintaining client privacy, dignity, and confidentiality
• Completing required documentation accurately and on time
