About The Position

The primary purpose of this job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves creating and maintaining a warm, positive, and calm environment, participating in team meetings, and ensuring resident comfort and safety. The CNA will also be responsible for accurate record-keeping, reporting any changes in resident condition, accidents, or incidents, and adhering to all facility policies and procedures regarding resident care and information privacy.

Requirements

• Must possess, as a minimum, a 10th grade education.
• Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
• Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
• CPR certification is required.

Responsibilities

• Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
• Identify residents before administering treatments or serving meals using wristbands or photo cards.
• Record all entries on flow sheets, notes, and charts in an informative and descriptive manner using authorized abbreviations.
• Report all changes in resident condition, accidents, and incidents to the Nurse Supervisor/Charge Nurse promptly.
• Maintain resident confidentiality and report any suspected or known violations of protected health information disclosure.
• Report any known or suspected unauthorized attempts to access the facility’s information system.
• Perform only trained nursing care procedures.
• Ensure resident rooms are prepared for new admissions, including making beds and having admission kits ready.
• Greet residents, escort them to their rooms, and introduce them to staff and other residents.
• Assist residents with packing personal possessions for transfers or discharges.
• Transport residents to new rooms or receiving areas, and assist with loading/unloading residents to/from vehicles.
• Create and maintain a warm, personal, and positive atmosphere, and a calm environment.
• Participate in shift nursing meetings to identify and correct problem areas and improve services.
• Report all resident complaints and grievances.
• Participate in and receive nursing reports.
• Follow established policies concerning exposure to blood/body fluids.
• Make beds, put extra covers on beds as requested.
• Measure and record resident's temperature, pulse, and respirations (TPRs), and weigh and measure residents as instructed.
• Ensure residents who are unable to call for help are checked frequently.
• Answer resident calls promptly and check residents routinely for personal care needs.
• Assist residents with identifying food arrangements and record food/fluid intake, reporting changes to the Nurse Supervisor/Charge Nurse.
• Keep resident water pitchers clean, filled, and within reach.
• Perform after-meal care, including removing trays and cleaning residents' hands, faces, and clothing.
• Follow work assignments and schedules.
• Cooperate with inter-departmental and facility personnel.
• Notify the facility when late or absent.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
• Assist residents with daily functions such as dental and mouth care, bathing, grooming, and dressing.
• Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
• Assist residents in preparing for medical tests and examinations.
• Assist with lifting, turning, moving, positioning, and transporting residents.
• Perform restorative and rehabilitative procedures as instructed.
• Check residents routinely to ensure personal care needs are met according to their wishes.
• Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers.
• Report injuries of an unknown source, including skin tears.
• Observe disoriented and comatose residents, recording and reporting data as instructed.
• Perform special treatments as instructed.
• Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
• Report defective equipment to the Nurse Supervisor/Charge Nurse.
• Report all allegations of resident abuse and/or misappropriation of resident property.
• Honor resident's refusal of treatment requests and report such requests to the supervisor.
