About The Position

The primary purpose of this job position is to provide each assigned resident with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as directed by supervisors. This role involves ensuring resident comfort, safety, and well-being through direct care and observation, while maintaining accurate records and adhering to facility policies and procedures. The position requires a compassionate individual who can create a warm and positive environment for residents.

Requirements

Must possess, as a minimum, a 10th grade education.
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
Ability to write reports, business correspondence, and procedure manuals.
Ability to effectively present information and respond to questions from managers and employees.
Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Must be a licensed Certified Nursing Assistant in accordance with laws of the state.

Responsibilities

Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
Identify residents before administering treatments or serving meals using wristbands or photo cards.
Use only authorized abbreviations when recording information.
Report changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
Record all entries on flow sheets, notes, and charts in an informative and descriptive manner.
Report all observed accidents and incidents as they occur.
Maintain resident confidentiality and report suspected violations of protected health information disclosure.
Report any unauthorized attempts to access the facility’s information system.
Perform only trained nursing care procedures.
Ensure resident rooms are ready for new admissions, including making beds and preparing admission kits.
Greet residents, escort them to their rooms, and introduce them to staff and other residents.
Make residents comfortable and assist with personal care needs such as bathing, dressing, and grooming.
Inventory, mark, and store resident’s personal possessions and clothing.
Assist residents with packing for transfers or discharge.
Transport residents to new rooms or receiving areas, and assist with loading/unloading from vehicles.
Create and maintain a warm, personal, and positive atmosphere, ensuring a calm environment.
Participate in regular meetings with nursing personnel to identify and correct problem areas and improve services.
Report all resident complaints and grievances.
Participate in and receive nursing reports.
Follow established policies concerning exposure to blood/body fluids.
Make beds, put extra covers on beds as requested.
Measure and record resident temperatures, pulses, and respirations (TPRs), and weigh and measure residents as instructed.
Ensure frequent checks for residents unable to call for help.
Answer resident calls promptly.
Assist residents with food arrangements and record food/fluid intake, reporting changes to the Nurse Supervisor.
Keep resident water pitchers clean, filled, and within reach.
Perform after-meal care, including removing trays and cleaning residents.
Follow work assignments and schedules.
Cooperate with inter-departmental and facility personnel.
Notify the facility when late or absent.
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
Assist residents with daily functions (dental and mouth care, bath functions, combing hair, dressing/undressing).
Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
Assist residents in preparing for medical tests.
Assist with lifting, turning, moving, positioning, and transporting residents.
Perform restorative and rehabilitative procedures as instructed.
Assist in preparing residents for physical examinations.
Observe and report skin breakdowns to prevent pressure ulcers.
Report injuries of unknown source, including skin tears.
Observe disoriented and comatose residents and record/report data.
Perform special treatments as instructed.
Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
Use only trained equipment and report defective equipment.
Maintain confidentiality of resident care information and report unauthorized disclosures.
Report all allegations of resident abuse and/or misappropriation of resident property.
Honor resident refusal of treatment requests and report them to the supervisor.
