About The Position

The primary purpose of this job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves maintaining a safe and comfortable environment for residents, accurately documenting care provided, and promptly reporting any changes in resident condition or any incidents. Key responsibilities include identifying residents before administering care, using authorized abbreviations in documentation, and maintaining resident confidentiality. The role also requires assisting residents with daily living activities such as bathing, dressing, eating, and mobility. Ensuring resident comfort, safety, and dignity is paramount. This position requires adherence to all facility policies and procedures, cooperation with inter-departmental personnel, and prompt reporting of any occupational exposures or concerns.

Requirements

Must possess, as a minimum, an 10th grade education.
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
Ability to write reports, business correspondence, and procedure manuals.
Ability to effectively present information and respond to questions from managers and employees.
Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
CPR certification required.

May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
Identify residents before administering treatments or serving meals.
Use only authorized abbreviations in all documentation.
Report all changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
Record all entries on flow sheets, notes, and charts in an informative and descriptive manner.
Report all observed accidents and incidents immediately.
Maintain resident confidentiality and report any suspected or known violations.
Report any known or suspected unauthorized attempts to access the facility’s information system.
Perform only trained nursing care procedures.
Ensure resident rooms are ready for new admissions.
Greet residents, escort them to their rooms, and introduce them to others.
Make residents comfortable and assist with personal belongings and storage.
Transport residents to new rooms or receiving areas, and assist with loading/unloading from vehicles.
Create and maintain a warm, positive, and calm environment.
Participate in shift nursing meetings to identify and correct problem areas.
Report all resident complaints and grievances.
Participate in and receive nursing reports.
Follow established policies concerning exposure to blood/body fluids.
Make beds and add extra covers as requested.
Measure and record resident temperatures, pulses, and respirations (TPRs).
Weigh and measure residents as instructed.
Ensure residents who are unable to call for help are checked frequently.
Answer resident calls promptly.
Check residents routinely to ensure personal care needs are met.
Assist residents with identifying food arrangements and record food/fluid intake.
Report changes in resident eating habits.
Keep resident water pitchers clean, filled, and within reach.
Perform after-meal care.
Follow work assignments and schedules.
Cooperate with inter-departmental and facility personnel.
Notify the facility when late or absent.
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
Assist residents with daily functions (dental care, bathing, grooming, dressing).
Keep residents dry by changing clothing and linens when wet or soiled.
Assist residents in preparing for medical tests.
Assist with lifting, turning, moving, positioning, and transporting residents.
Perform restorative and rehabilitative procedures as instructed.
Assist in preparing residents for physical examinations.
Observe and report skin breakdowns to prevent pressure ulcers.
Report injuries of unknown source, including skin tears.
Observe disoriented and comatose residents and record/report data.
Perform special treatments as instructed.
Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
Use only trained equipment and report defective equipment.
Report all allegations of resident abuse and/or misappropriation of resident property.
Honor resident refusal of treatment requests and report them to a supervisor.
