About The Position

This is a full-time Certified Nursing Assistant (CNA) position with Riverside Home Health & Hospice in Southern Oregon, serving Medford, Grants Pass, and surrounding areas. The role focuses on providing compassionate, patient-centered hospice care in patients' homes. CNAs are essential in offering comfort, dignity, and quality of life during meaningful moments for patients and their families. The position emphasizes presence, patience, and connection, going beyond task-oriented care to provide emotional support and identify changes in patient condition.

Requirements

• Current Oregon CNA license
• CPR Certification
• Valid driver’s license and reliable transportation
• Strong communication and observation skills
• Ability to work independently in the field

Responsibilities

• Provide personal, hands-on care in patients’ homes
• Help patients maintain dignity, comfort, and quality of life
• Support families during emotional and meaningful moments
• Act as the eyes and ears of the care team, identifying changes in condition
• Help patients remain safely and comfortably at home
• Provide personal care (bathing, dressing, toileting, hygiene)
• Assist with mobility and range of motion exercises
• Take and record vital signs
• Maintain a clean and safe patient environment
• Report changes in condition to the nurse or case manager
• Support patients and families with compassion and respect
