About The Position

UPMC Home Health and Hospice is seeking a Home Care Aide to join their team in Altoona, Blair, and surrounding areas. In this role, you will work in a home setting with a diverse patient population, providing care in the comfort of their homes. You will function as an extension of skilled nursing and/or rehabilitation services, providing designated care under the direction of the primary service provider to achieve identified goals. This position offers a daylight schedule with weekend and holiday rotation, and no on-call duties. Mileage reimbursement and full-time benefits are provided.

Requirements

  • High School Diploma or GED - Required
  • Minimum of 75 hours of Nurse Aide training – Required. If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

Nice To Haves

  • One (1) year experience - Preferred
  • Certified Nurse Aide – Preferred
  • Nurse Aide Registry in good standing either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.

Responsibilities

  • Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Proven ability to facilitate change.
  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
  • Participates in all required agency meetings.
  • Must maintain annual competency evaluation scores as determined by regulatory requirements.
