Requirements
- Valid NYS Certified Nursing Assistant License (CNA) - Required
- Ability to work in a team oriented work group
- Proficient in various computer programs and Medical Records Documentation system
Nice To Haves
- Minimum of 1 year experience working with Elders - Preferred
Responsibilities
- Provide high quality, individualizes patient care in collaboration with our nursing team
- Take initiative while assisting and maintaining patient care
- Assist the resident in activities necessary as per individualized care planning
- Transport Elders to and from activities
- Perform evaluation and reporting
- Provide resident and family with emotional support individualized care planning
- Receive/give reports to team members/team leaders to appropriate professional staff
