About The Position

Community Care Hospice, a partnership of Midland Care Connection, provides compassionate, personalized end-of-life care to individuals and their families in central and south-central Kansas. As a not-for-profit organization with decades of experience, Midland Care focuses on comfort, dignity, and support for those facing life-limiting illnesses, delivering services tailored to each person’s needs at home or in partner facilities. Through the Community Care Hospice joint venture with local nonprofit retirement and senior living communities, the program extends this expertise across multiple counties, combining skilled clinical care with emotional and spiritual support to help patients and families navigate the final stages of life with compassion and respect.

Requirements

  • Current Certified Nurse Aide certification
  • Willingness to enroll in HHA certification course within the first 90 days of employment
  • Ability to provide companionship and assist with daily living activities (e.g., mobility, meal support, safety) in clients’ home
  • Help clients stay safe and comfortable in their own homes

Nice To Haves

  • At least one year of experience working as a CNA in hospice, home health, long-term care, or assisted living setting
  • CPR and First Aid certification
  • Strong communication and interpersonal skills

Responsibilities

  • Provides hands-on personal care and comfort support to patients receiving end-of-life care at home.
  • Assists with bathing, grooming, dressing, toileting, turning, and repositioning to maintain comfort and prevent skin issues.
  • May help with meals, feeding, light housekeeping, laundry, and changing bed linens.
  • Observes the patient’s condition and reports any changes to the hospice nurse.
  • Maintains dignity, comfort, and quality of life.
  • Gives family caregivers practical support and relief.
  • Travel within the community to deliver care and support.
