About The Position

The primary purpose of this job position is to provide each assigned resident with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as directed by supervisors. This role involves ensuring resident comfort, safety, and well-being through direct care and observation, while adhering to facility policies and procedures. The position requires maintaining a positive and warm environment, accurate record-keeping, and effective communication with residents, supervisors, and other personnel.

Requirements

  • Must possess, as a minimum, a 10th-grade education.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
  • Occasionally lift and/or move up to 25 pounds.
  • Prolonged use of a desktop or laptop computer.
  • Regularly required to sit, stand, walk, talk, read, or hear.
  • Frequent use of all office-related equipment to include copier/scanner/fax, telephone, and calculator.

Responsibilities

  • Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
  • Identify residents before administering treatments or serving meals using wristbands or photo cards.
  • Record all entries on flow sheets, notes, and charts in an informative and descriptive manner, using only authorized abbreviations.
  • Report all changes in resident condition, accidents, and incidents to the Nurse Supervisor/Charge Nurse promptly.
  • Maintain resident confidentiality and report any suspected or known violations of protected health information or unauthorized attempts to access facility systems.
  • Perform only trained nursing care procedures and ensure resident rooms are prepared for new admissions.
  • Greet residents, escort them to their rooms, and introduce them to others as appropriate.
  • Assist residents with personal care needs, including bathing, dressing, grooming, and toileting.
  • Assist residents with packing and storing personal possessions during transfers or discharges.
  • Transport residents to new rooms or receiving areas, and assist with loading/unloading from vehicles.
  • Create and maintain a warm, positive, and calm environment for residents.
  • Participate in shift nursing meetings to identify and correct problem areas and improve services.
  • Report all resident complaints and grievances to supervisors.
  • Participate in and receive nursing reports at the beginning and end of shifts.
  • Follow established policies concerning exposure to blood/body fluids and report occupational exposures.
  • Make beds, provide extra covers, and ensure residents are kept dry.
  • Measure and record resident vital signs (temperature, pulse, respiration) and weight/height as instructed.
  • Ensure residents unable to call for help are checked frequently and answer calls promptly.
  • Assist residents with identifying food arrangements and record food/fluid intake, reporting changes in eating habits.
  • Keep resident water pitchers clean, filled, and within reach.
  • Perform after-meal care, including removing trays and cleaning residents.
  • Follow work assignments and schedules, cooperating with inter-departmental personnel.
  • Notify the facility when late or absent from work.
  • Assist residents with daily functions, including dental and mouth care, bathing, and dressing.
  • Assist residents in preparing for medical tests and physical examinations.
  • Check residents routinely to ensure personal care needs are met according to their wishes.
  • Observe and report skin breakdowns, pressure areas, and injuries of unknown source to prevent complications.
  • Observe disoriented and comatose residents and record/report data as instructed.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
  • Use only trained equipment and report defective equipment.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident refusal of treatment requests and report them to supervisors.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in the evacuation of residents during emergency situations.
