About The Position

Allay Hospice is seeking a dedicated Hospice Certified Nursing Assistant to provide dignified, compassionate care and support to patients and families. The role provides one-to-one care in patients’ homes across a territory primarily covering Waukesha and Milwaukee Counties, with travel required. CNAs work independently while receiving team support and collaborate with the hospice interdisciplinary team. The schedule is full-time, Monday through Friday, 8:00 a.m. to 5:00 p.m., with an office location at 325 N Corporate Dr, Suite 260, Brookfield, WI 53045.

Requirements

• Meet the training requirements of the state of practice.
• At least six months of experience, including providing personal care.
• Be at least 18 years of age.
• Complete at least 75 hours of classroom and supervised practical training; 16 classroom hours must be completed before the required 16 hours of practical training.
• Possess and maintain current CPR certification.
• Ability to read and follow written instructions and document care provided.
• Understand the hospice philosophy and be comfortable providing specialized care to terminally ill patients.
• Ability to work with little direct supervision.
• Ability to handle death and dying.
• Strong oral and written communication skills.
• Good organizational skills.

Responsibilities

• Assist patients with bathing, dressing, toileting, mobility, and repositioning.
• Observe and report changes in patients’ conditions.
• Assist with ambulation, transfers, range-of-motion exercises, and safe equipment use.
• Assist with self-administered medications in accordance with state regulations.
• Offer companionship and comfort to patients and families.
• Change linens, assist with meals, and help maintain a clean space.
• Maintain accurate and timely documentation.
• Collaborate with the hospice interdisciplinary team.
