About The Position

Midland Care Connection is seeking a Certified Nursing Assistant (CNA) to provide essential, hands-on care to clients in their homes, enabling them to remain safe, healthy, and independent. This role is crucial for supporting older adults and individuals with chronic conditions in a familiar home environment. The organization is a not-for-profit, community-based entity established in 1978, offering a full continuum of care including hospice, palliative care, home health, grief and loss counseling, and a PACE program. Their mission is to 'meet individual needs through innovative care solutions' across multiple counties in northeast Kansas.

Requirements

• Current Certified Nursing Assistant (CNA) certification in the state of Kansas
• High school diploma or GED
• Ability to provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility
• Basic knowledge of infection control, safety practices, and patient rights
• Ability to observe, document, and report changes in a client’s condition to nursing staff or supervisors
• Compassionate, respectful, and patient-centered approach to care
• Strong communication and interpersonal skills
• Ability to work independently in clients’ homes while following care plans
• Physical ability to perform job duties, including lifting, bending, and standing for extended periods
• Valid driver’s license, reliable transportation, and willingness to travel to clients’ homes
• Ability to pass background checks, health screenings, and required trainings

Nice To Haves

• Previous experience working with older adults, individuals with chronic illness, or those with mobility or cognitive impairments
• Prior home health, hospice, palliative care, or PACE program experience
• Experience assisting with light housekeeping, meal preparation, and medication reminders
• Familiarity with interdisciplinary care teams and community-based healthcare models
• CPR/First Aid certification
• Demonstrated ability to provide emotional support and companionship to clients

Responsibilities

• Assisting with activities of daily living such as bathing, dressing, grooming, toileting, and mobility.
• Helping with basic household tasks, meal preparation, and medication reminders.
• Monitoring and reporting changes in a client’s condition to nurses or supervisors.
• Supporting client comfort and safety.
• Offering companionship to clients.

Benefits

• PRN pay rate: CNA - $18.15+/hour | HHA - $20.15+/hour
• Bonuses: CNA - $1,000 | HHA - $1,500 (paid in two increments at 6 months and 1 year of employment)
• Paid training for CNAs to obtain their HHA license after 60 days
• Shift differentials: $1.00 Weekdays (5:00pm – 8:00am), $1.50 Weekends (8:00am – 5:00pm), $2.00 Weekends (5:00pm – 8:00pm)
