About The Position

Family Health Centers of San Diego (FHCSD) is seeking a Certified Nursing Assistant (CNA) for their Program of All-Inclusive Care for the Elderly (PACE) program. This role is crucial in providing direct care to participants, ensuring their satisfaction and retention, and adhering to all regulatory requirements. The CNA will assist participants with instrumental Activities of Daily Living (IADLs) in various settings, including their homes, and will perform personal care tasks. The position also involves helping participants with mobility, identifying safety hazards, measuring vital signs, observing and reporting changes in participant status, and participating in care conferences. Additionally, the CNA will assist with maintaining the cleanliness and order of the PACE Center, including the dining area and activities.

Requirements

  • High School diploma or equivalent GED Certificate required. Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
  • Possess a California Certified Nurse Assistant Certificate required.
  • Current American Heart Association Heartsaver CPR AED Certification required within 90 days of employment.
  • DRIVING IS AN ESSENTIAL JOB FUNCTION & DRIVES COMPANY VEHICLE
  • Requires an active California driver's license for the class required for operation of assigned vehicle
  • Traveling between sites and other locations is required as an essential function of the job.
  • Must have a car, a valid California driver’s license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended.
  • An acceptable driving record is also required.
  • Able to work effectively with culturally, economically and educationally diverse populations.
  • Demonstrated positive interpersonal skills in dealing with a wide range of staff and/or a patients’ essential forms of living.
  • Able to perform a wide range of duties in caring for the older adult population.
  • Able to assist with personal care in homes and/or at health-related facilities to include wheelchair and ambulatory transfer between home and various transportation modes.
  • Able to handle difficult situations in a professional manner.
  • Strong interpersonal skills with the ability to get along well with others and gain cooperation.

Nice To Haves

  • Experience working with the elderly in a skilled nursing facility, residential care facility, hospital setting, or within a home setting preferred.
  • Bilingual Spanish/English preferred.

Responsibilities

  • Provide care in a manner which ensures participant satisfaction and retention, and is in compliance with all regulatory requirements and guidelines.
  • Assist with instrumental Activities of Daily Living (IADLs) across care settings.
  • Assist participant in their home with light housekeeping, medication reminder, and meal preparation.
  • Provide personal care to include bathing, dressing, toileting, oral and denture care, hair and skin, shaving, nail trimming, etc.
  • Help participant’s to move, turn, get in and out of bed, chair or wheelchair, and ambulate with or without assistive devices.
  • Provide physical assistance and attendant care across care settings.
  • Identify safety hazards and emergency situations. Initiate corrective action as appropriate and report situation to charge person/supervisor.
  • Measure and record vital signs/weights and reports information to appropriate medical staff following established procedures and protocols.
  • Observe and report to immediate supervisor any changes in participant’s physical or mental status; record such observations on appropriate records.
  • Participate in case conferences with other members of the interdisciplinary care team as appropriate.
  • Help maintain order and cleanliness of the multi-purpose PACE Center.
  • Assist with dining area set-up and distribution of meals and snacks as assigned.
  • Assist with multi-purpose PACE Center activities as assigned.
  • Perform other duties as assigned.
