About The Position

Professional Case Management (PCM) is hiring Certified Nursing Assistants (CNAs) to provide in-home healthcare services to help those who have served our country. Requirements

Active certification as a CNA in the state(s) of practice.
Current CPR certification.
Current TB test.
Food Handler's card.

Responsibilities

Provides personal care services under the direction of the RN/Case Manager.
Assigned to specific clients and performs services for clients as necessary to maintain their personal comfort.
Perform skilled transfers.
Monitor vital signs.
Complete timely and accurate documentation.
Assist with range of motion exercises.
Assist with all activities of daily living including light duty housekeeping, meal preparation and patient hygiene.
