About The Position

Tapestry Hospice & Palliative Care is seeking exceptional Certified Nursing Assistants (CNAs) to join their growing family. The role involves providing compassion, comfort, and peace to patients and their families during life's most important moments. The CNA will be supported by an experienced interdisciplinary team and will be a key member of the care team, serving as the eyes, ears, and heart of patient care. This position requires providing personal care services, observing patient condition, and communicating changes to the Registered Nurse Case Manager, while following individualized care plans and helping patients maintain dignity and comfort.

Requirements

• Must live in Murray, Gordon, or Gilmer County, Georgia
• Current Georgia CNA Certification
• Current CPR & First Aid Certification
• Valid Driver's License, Reliable Vehicle & Auto Insurance
• Ability to travel throughout assigned territory
• Compassionate, dependable, and patient-focused

Nice To Haves

• Hospice, home health, assisted living, or long-term care experience preferred
• 1-2 years of hospice experience preferred
• Candidates with experience in home health, assisted living, skilled nursing, or long-term care are encouraged to apply
• Strong communication and time management skills
• Genuine passion for serving patients and families

Responsibilities

• Provide personal care and assistance with activities of daily living
• Follow individualized patient care plans
• Observe and report patient changes to the RN Case Manager
• Document care accurately and timely
• Support patients and families with compassion and professionalism
• Maintain infection control and patient safety standards
• Collaborate with nurses, social workers, chaplains, and other care team members
• Travel to patient homes within the assigned service area
