Primary Work Location: MULTISITE: Multisite staff work across several program locations throughout Alameda County, providing care and support wherever it is most needed. These sites offer a range of services, including post-hospitalization medically supported shelter, medical respite care, skilled nursing, and caregiving support. Job Summary: The CNA provides essential care and support to patients under the supervision of registered nurses (RNs) or licensed practical nurses (LVNs). Responsibilities include assisting patients with activities of daily living such as bathing, dressing, and grooming, monitoring vital signs, feeding patients, and helping with mobility. CNAs also provide emotional support to patients and report any changes in patient condition to the nursing staff. They play a crucial role in ensuring patients’ comfort, safety, and overall well-being in healthcare settings. New graduates are encouraged to apply. Cardea Health is also seeking candidates who are willing to obtain their Home Health Aide (HHA) license with our support, which will be accompanied by a slight pay increase upon certification. Cardea Health staff collaborate with other housing programs and social services to create a safe, independent living environment for a dynamic and vulnerable population. The Housing Coordinator will need to be flexible, patient, and willing to work in challenging environments with a diverse population. Experience working with homeless or formerly incarcerated populations is highly desirable. Requirements

  • High School Diploma or equivalent
  • Must be authorized to live and work in the U.S.
  • Must be 18 years of age or older
  • Must have personal, reliable transportation to and from work sites and shifts
  • Compliance with all company policies
  • Must provide proof of COVID vaccination or willing to be vaccinated

Nice To Haves

  • Experience with homeless or incarcerated populations is highly desirable
  • Two years of experience in patient care
  • Self-directed and able to work with minimal supervision
  • Detailed oriented and highly organized
  • Prior experience with homeless or incarcerated populations is highly desirable
  • Proficiency in Microsoft Outlook, Word and Excel is required

Responsibilities

  • The CNA will assist clinical staff and residents in the coordination of medical and behavioral health care.
  • Conduct patient/personal care in accordance with resident plan of care
  • Support resident relationship with primary care and other professional disciplines
  • Assist nursing team in communication with residents including arranging and facilitating appointments with nursing staff
  • Ensure adherence to infection control standards on site among staff and residents
  • In collaboration with housing agencies, ensure daily wellness of resident and participate in immunization programs
  • Assist in the execution of population health and safety interventions on site
  • Answer calls to nursing station and respond independently to when appropriate
  • Assist nursing staff with light clerical duties including filing and making copies
  • Ensure that clinical documentation is current, timely, and accurate
  • Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA)
  • Recognize and respond to medical emergencies on site, follow up with MD communication or 911 activation
  • Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect
  • Other duties and responsibilities as required
