About The Position

Mountain Valley provides personalized hospice care to patients and their families during life’s most challenging moments. We are currently seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join our team of dedicated professionals in delivering exceptional care. Work alongside a team of supportive nurses, social workers, chaplains, and providers who share a deep commitment to quality care and service.

Requirements

• Certified Nursing Assistant (CNA) with current state licensure.
• Ability to communicate clearly and empathetically with patients, families, and the interdisciplinary team.
• Ability to work independently and manage time effectively, with a commitment to delivering excellent care.

Nice To Haves

• Experience is preferred, but we welcome candidates with a passion for compassionate care and a desire to make a difference.

Responsibilities

• Assist patients with daily living activities such as bathing, dressing, grooming, feeding, and mobility.
• Monitor patients for changes in condition, ensuring they are comfortable in their home environment – this could be their home, skilled nursing facility or assisted living facility.
• Build meaningful relationships with patients and their families, providing compassionate care.
• Work closely with nurses, social workers, and chaplains to deliver holistic care that meets each patient’s needs.
• Document care provided, changes in patient condition, and other important details to ensure seamless communication with the care team.
