About The Position

Our Hospice Agency is based in Ottumwa, Iowa, and provides hospice services throughout the following Iowa counties: Appanoose, Davis, Jefferson, Keokuk, Mahaska, Marion, Monroe, Van Buren, Warren, and Wapello. Additional position highlights include: Monday through Friday schedule, 8:00 a.m. to 4:30 p.m. Supported by 24/7 on-call nursing coverage Opportunity to serve patients and families across a large rural service area with an established interdisciplinary hospice team. The Certified Nurse Practitioner, in collaboration and consultation with physicians, staff RNs and other health care professionals, provides holistic, compassionate care to client and families. The Nurse Practitioner practices within the context of collaborative management with a physician(s) in diagnosing, managing, and preventing acute and chronic illness and disease, and promoting wellness. The Nurse Practitioner must have an advanced nursing practice that includes independent nursing functions based on the applicable Scope of Practice, nursing standards of care and a role in medical management within a collaborative agreement with a physician(s).

Requirements

• Completion of a master’s, postmaster’s, or doctorate from an nurse practitioner program accredited by the Commission on the Collegiate of Nursing Education or National League for Nursing Accrediting Commission.
• Minimum two years clinical experience as a Nurse Practitioner in geriatric, adult or family practice setting.
• Demonstrated current competence and provision of care, treatment, or services for an adequate volume of patients in the past twelve months, or completion of master’s/post-master’s degree program in the past twelve months.
• Experience must correlate to the privileges requested.
• Current licensure by the applicable state board of nursing for advanced practice.
• Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances.
• Current certification by the American Nurses Credentialing Center or an equivalent nationally recognized body.
• Submit current Scope of Practice that has been approved by the applicable state Board of Nursing for advanced practice.
• Obtains and subsequently maintains required department specific competencies and certifications.

Nice To Haves

• Licensing prior to August 1, 1995 in ND, master's degree in nursing is preferred.
• Experience with palliative care or hospice preferred.

Responsibilities

• Performs in-depth health risk assessments on all assigned client.
• Works collaboratively with physicians, case managers, social workers, family members, key care givers and any appropriate ancillary medical personnel as appropriate.
• Responsible for identifying overall quality improvement activities through assessing and planning health care which is individualized to the client and consistent with policies and protocols.
• Reviews and interprets the medical, surgical and social history of client.
• Performs appropriate screening procedures for client.
• Interprets laboratory and other diagnostic data and recommends appropriate treatments or actions for follow-up, reconciles medications.
• Responsible for prescribing or performing various treatment modalities including, but not limited to: medications, injections, wellness counseling, suturing and ancillary testing.
• Assists hospice location employees in maximizing client quality of life.
• Evaluates clients who have acute changes in condition, and collaborates with the primary care provider and employees to implement appropriate interventions.
• Assists in meeting regulatory requirements related to timely client certification for services.
• Collaborates with the interdisciplinary team to create, review, and revise the client's plan of care.
• Collaborates with HCBS admission team to support open access for the client to agency services.
• Provides medical expertise on pain, symptom management, and the evaluation of terminal prognosis to clients, family members and staff.
• Is compliant with Occupational Safety and Health Administration (OSHA), State Board of Nursing.
• Participates in all mandatory compliance and other training programs, seeks guidance for compliance-related concerns and adheres to all applicable laws, regulations, policies and procedures.
• Refers clients to primary care provider (PCP,) case management and disease management programs as necessary.
• Practices in accordance with applicable local, state, and federal laws which govern confidentiality and medical information privacy regulations (HIPAA).
• Maintains control of all PHI information.
• Consistently identifies unmet needs and gaps in care, and initiates appropriate referrals as indicated.
• Synthesizes information and easily diagnoses and creates a comprehensive plan of care.
• Achieves clinical performance targets.
• Creates positive rapport and engagement with client.
• Participates in different roles or interdisciplinary teams as assigned.
• Assists on special project as assigned.
• Participates in client meetings as requested.
• Communicates and collaborates with Facility leadership to achieve client care needs and goals.
• Performs other duties as assigned.
