About The Position

The CNA/QMAP supports clinic operations by providing direct participant care, medication administration (within scope as a Qualified Medication Administration Person), and assisting the clinic team to ensure safe, efficient, and high-quality care. This role combines hands-on clinical support with medication administration and patient flow coordination in a clinic setting. PACE (Program of All-inclusive Care for the Elderly) is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes.

Requirements

• Minimum of 2 years’ experience as a CNA required.
• Minimum of 2 years’ experience as a QMAP required.
• Current Certified Nursing Assistant (CNA) certification in good standing in the state of Colorado required.
• Current QMAP certification in the State of Colorado required.
• CPR Certification is maintained throughout employment.
• Knowledge of basic nursing care and infection control practices
• Understanding of medication administration protocols within QMAP scope
• Strong attention to detail and documentation accuracy
• Effective communication and interpersonal skills
• Ability to multitask and work in a fast-paced clinic environment
• Basic computer skills and familiarity with EHR systems

Nice To Haves

• Previous clinic, outpatient, or long-term care experience preferred.
