About The Position

Hospice Certified Nurse Assistant/Home Health Aide, (Enfield Region) Location: Enfield Region Status: Full Time Bonus: Sign On Bonus Trinity Health Of New England At Home serves patients and their loved ones by providing a variety of in-home services including but not limited to nursing, social work and physical/occupational/speech therapies. Position Purpose: Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures. Position Details: Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year. Coverage Region: Enfield Bonus: Position qualifies for a $5,000 sign-on bonus paid out over two years. NOTE: This position is 100% home care providing services in patients’ private homes.

Requirements

• High School diploma or GED required.
• Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program (CNA license for CT required)
• Documentation of one year’s employment as an aide in an institution, home care, or hospice setting.
• Meets agency hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)
• Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency’s client and their family.
• Must accept responsibility for maintaining skills, learning and adhering to agencies policies and procedures on an on-going basis.
• Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics, and adhere to the Compliance Program.
• Must have current Driver’s license and reliable transportation to and from work site.

Responsibilities

• Follows care plan developed by RN.
• Reports client changes in condition to RN, as described on care plan.
• Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
• Assists clients with household tasks including, planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
• Measures and records client’s oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.
• Performs range of motion and other simple procedures as an extension of therapy services as assigned.
• Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record.
• The Home Health / Hospice Aide shall not administer medication of any kind.
