PACE (Program of All-Inclusive Care for the Elderly) is a unique program that helps individuals 55 and older who qualify for nursing facility-level care remain in their homes. We provide care at our state-of-the-art center and in participants' homes. Serving the community since 2019, PACE North is expanding rapidly! What You’ll Do: As a vital part of our home care team, you’ll provide compassionate care to our participants in their homes. We have participants living within a 7 county area with Grand Traverse at the center. Your responsibilities will include: · Assisting with daily living activities in the home such as bathing, dressing, cleaning, shopping and meal support. · Taking vital signs and documenting participant care. · Supporting our interdisciplinary team to ensure high-quality care.
