About The Position

The Certified Nursing Assistant (CNA) is responsible for ensuring the safety, supervision, and well-being of individuals receiving services while implementing each individual's care plan. The CNA provides direct personal care and support with activities of daily living (ADLs) in residential, community, and healthcare settings. This position works collaboratively with the Registered Nurse (RN), Program Director, and interdisciplinary team to promote health, safety, independence, and quality of life for the individuals served. The CNA also assists with healthcare coordination, transportation, documentation, environmental safety, and administrative duties while demonstrating professionalism and a commitment to the organization's mission, values, and person-centered approach to care.

Requirements

  • Current Certified Nursing Assistant (CNA) certification in good standing with the applicable State Board of Nursing.
  • High School Diploma or GED.
  • Valid driver's license.
  • Reliable transportation.
  • Demonstrated safe driving record and ability to meet organizational driving requirements.
  • Ability to successfully complete all required training and certifications.
  • Ability to provide compassionate, person-centered care and support individuals with personal care and daily living needs.
  • Ability to maintain a safe environment and appropriately respond to medical, behavioral, or emergency situations.
  • Ability to communicate effectively with individuals served, families, healthcare providers, and team members.
  • Ability to work cooperatively with the Registered Nurse, Program Director, and interdisciplinary team.
  • Ability to accurately complete service documentation, maintain records, and follow reporting requirements.
  • Ability to work across multiple settings and adjust to changing consumer and program needs.
  • Ability to perform physically demanding tasks including lifting, transferring, standing, walking, and active supervision.
  • Ability to manage competing responsibilities while ensuring quality care and timely completion of assigned tasks.
  • Follow all OSHA regulations and workplace safety requirements.
  • Adhere to all Fire Safety and Emergency Procedures.
  • Comply with all HIPAA confidentiality and privacy standards.
  • Follow organizational Human Resources, Fiscal, and Administrative policies.
  • Complete required training programs and maintain all required certifications.
  • Demonstrate professionalism, integrity, and sound judgment at all times.
  • Maintain awareness of Bloodborne Pathogens, Right-to-Know regulations, security procedures, and workplace safety practices.

Responsibilities

  • Provide direct support and supervision to individuals in accordance with their Individual Support Plan (ISP), care plan, and organizational policies.
  • Assist individuals with activities of daily living (ADLs), including: Bathing, Grooming, Dressing, Toileting, Oral hygiene, Eating and meal support.
  • Administer medications and prescribed treatments in accordance with physician orders, agency policies, and applicable regulations.
  • Support individuals with mobility needs, including lifting, transferring, positioning, and ambulation assistance.
  • Monitor and report changes in physical, emotional, or behavioral health status.
  • Implement approved interventions during behavioral or medical crises, including supporting individuals exhibiting self-injurious or aggressive behaviors.
  • Participate in and perform physical supports and interventions consistent with Crisis Prevention Intervention (CPI) and agency training requirements.
  • Accompany and assist individuals during community outings, appointments, recreational activities, employment activities, and social engagements.
  • Provide services in multiple environments including residential homes, community settings, hospitals, healthcare facilities, and other designated locations.
  • Maintain a safe, clean, sanitary, and healthy living environment for individuals receiving services.
  • Monitor the environment for health, safety, and maintenance concerns and promptly report issues to appropriate personnel.
  • Follow all infection control, bloodborne pathogen, health and safety, and emergency preparedness procedures.
  • Collaborate with the Registered Nurse and Program Director to promote consumer health and environmental safety.
  • Maintain required staffing ratios and supervision levels as outlined in individual support plans and program requirements.
  • Complete accurate, timely, and thorough documentation related to: Individual Support Plan (ISP) goals, Progress notes, Daily services provided, Behavioral observations, Health-related concerns, Incidents and outcomes.
  • Assist with assessing individual skill development and monitoring progress toward goals.
  • Participate in ISP implementation, program planning, and team meetings as required.
  • Maintain confidentiality of all consumer, organizational, and employee information in accordance with HIPAA and agency policies.
  • Collaborate effectively with the RN, Program Director, and multidisciplinary team members.
  • Perform administrative duties and other assigned tasks that support program operations.
  • Attend all required staff meetings, trainings, certifications, and professional development activities.
  • Maintain a professional demeanor and sound judgment in all aspects of employment.
  • Support organizational values, policies, and procedures while promoting a positive and respectful work environment.
  • Perform other duties as assigned to support the effective operation of the program.
