About The Position

The Certified Nurse Aide (CNA) will work in the Adult Day Health Care department. This role involves overseeing registrants during daily activities, assisting them with Activities of Daily Living (ADLs), and providing personal care. The CNA will also be responsible for initiating activity programs, documenting registrant information, observing and reporting changes in registrants, and performing nursing aide duties on other units as needed. Adherence to ethical standards, corporate compliance, and confidentiality policies is required.

Requirements

  • High school diploma or equivalent.
  • Successful completion of a Certified Nurse Aide training program or enrollment in an accredited licensed nursing program.
  • Valid New York State Class 5 or "D" license with clean driving record.
  • Follows written and verbal instructions.
  • Completes mandatory inservices and departmental inservices.
  • Reviews registrant care plans per schedule.
  • Documents necessary information in the registrant records.
  • Observes and reports changes in registrant behavior, attitudes and/or physical capabilities to the nurse and/or supervisor.
  • Completion of documentation (medical flow sheets, vital signs, etc.) as assigned.
  • Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws.
  • Adheres to the facility's confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees, as necessary to perform job duties.

  • Geriatric experience preferred.

Responsibilities

  • Oversee registrants as they follow their daily scheduled activities and assist with completion of ADLs.
  • Assist registrants as they arrive and leave the program and to specific areas (e.g., OT, PT, dining room, activities, dental, and beauty-barber shop).
  • Initiate activity programs within the department in conjunction with supervisor and/or recreation leader.
  • Give personal care assistance, e.g., baths/showers.
  • Lift or bear weight up to 100 lbs. when transferring registrants.
  • Travel to and from home of registrants to pick up/drop off, if necessary (door-to-door assistance).
  • May drive facility vehicles to transport registrants to and from home or activity programs.
  • Follow written and verbal instructions.
  • Complete mandatory inservices and departmental inservices.
  • Review registrant care plans per schedule.
  • Document necessary information in the registrant records.
  • Observe and report changes in registrant behavior, attitudes and/or physical capabilities to the nurse and/or supervisor.
  • Perform nursing aide duties on other units (i.e., rehab or SNF houses) as needed.
  • Complete documentation (medical flow sheets, vital signs, etc.) as assigned.
  • Perform duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws.
  • Adhere to the facility's confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees, as necessary to perform job duties.
  • Perform any other duties as assigned by Program Director or designee.
