About The Position

Community Healthcare of Texas provides hospice and palliative care services for individuals living with serious illnesses and supports their loved ones. The Certified Nurse Aide provides personal hygiene care to hospice patients according to each patient’s Plan of Care. Depending on census, assignments may include patients in the field or in inpatient units.

The role requires a high school degree or GED, current proof of state Certified Nurse Aide certification, and one year of direct patient care experience. The aide must demonstrate skills and written competency before working independently with a patient. The position is scheduled Monday through Friday, 8:00 a.m. to 5:00 p.m.

Requirements

Responsibilities

• Provide personal hygiene care according to the patient’s individual Plan of Care, including bathing and shampooing.
• Deliver routine medical supplies to patients as needed.
• Observe and report changes in a patient’s condition to the Nurse/Care Manager.
• Demonstrate respect for and understanding of CHOT’s philosophy, mission, and service standards, and provide excellent customer service.
• Complete 12 hours of in-service training annually.
• Assist patients with transfers as needed.
• Provide compassionate companionship while delivering services.
• Serve patients in the field or in inpatient units, depending on census.
