This role involves utilizing the nursing process to provide patient care and complete documentation. Responsibilities include administering nursing assessments, contributing to and implementing patient care plans, and responding promptly to patient needs. The Certified Nurse Aide will monitor changes in patient condition, notify the charge nurse, and act as a patient advocate. Tasks include taking vital signs, assisting with Activities of Daily Living (ADLs), and performing Glucose Point-of-Care testing. The position requires active participation in the Care Team, effective communication of patient concerns, and adaptability to workload changes while maintaining quality care. The aide will also be responsible for initiating and completing duties, promoting teamwork, and demonstrating initiative and flexibility. Regular patient observation and documentation of findings for the Charge Nurse are essential, along with prompt reporting of any changes or adverse reactions. The role includes helping to set care priorities, managing equipment and supplies, and reporting equipment needing repair. Accurate and ongoing documentation of patient status, interventions, responses, teaching, and discharge outcomes in the care tracker is required, along with completing shift-end checks. Effective communication, including following approved shift report formats and collaborating with health team members, is crucial. Promoting patient and staff safety through proper body mechanics, transfer/lifting techniques, and adherence to infectious disease controls, including isolation procedures and proper use of hand washing and gloves, is a key aspect of the role. Other duties as assigned will also be performed.
