About The Position

This role involves utilizing the nursing process to provide patient care and complete documentation. Responsibilities include performing nursing assessments, contributing to and implementing patient care plans, responding to patient needs promptly, recognizing changes in patient condition, and serving as a patient advocate. The position also requires participating as an effective member of the Care Team, responding to patient concerns, providing written communication of concerns, adapting to workload changes, and promoting teamwork. Additionally, the role involves observing patients, documenting information for the Charge Nurse, helping to set priorities in nursing care, maintaining knowledge of equipment and supplies, performing and maintaining documentation accurately, communicating effectively using approved shift report formats, and promoting patient and staff safety through proper body mechanics and infectious disease controls. Other duties as assigned.

Requirements

  • Reports equipment needing repairs.
  • Cleans and stores equipment.
  • Performs and maintains documentation.
  • Documents accurate and ongoing assessments.
  • Completes shift end check.
  • Communicates effectively.
  • Follows approved shift report format.
  • Identifies and recognizes abnormal symptoms/changes.
  • Takes appropriate action for patient condition changes.
  • Communicates and collaborates effectively with health team members.
  • Promotes and ensures patient and staff safety.
  • Utilizes proper body mechanics, transfer/lifting techniques.
  • Implements infectious disease controls.
  • Proper use of hand washing and gloves.

Responsibilities

  • Utilizes the nursing process for patient care and documentation.
  • Administers and performs nursing assessments for various patient types (Acute, Observation, ER, SNF, Outpatient).
  • Contributes to, monitors, and implements patient’s plan of care.
  • Responds to patient needs in a timely fashion.
  • Recognizes changes in patient's condition and notifies the charge nurse.
  • Serves as a patient advocate.
  • Performs tasks such as vital signs, ADLs, and Glucose POC.
  • Participates as an effective member of the Care Team.
  • Responds in a timely and effective manner to patient concerns and provides immediate interventions.
  • Provides written communication of concerns to necessary departments.
  • Adapts to changes in workload and completes assignments while maintaining quality care.
  • Initiates and performs duties that need to be completed although not directly assigned.
  • Promotes teamwork through effective interpersonal relationships.
  • Demonstrates initiative and flexibility in the provision of care.
  • Offers assistance to other team members as needed.
  • Observes the patient at regular intervals and documents information for the Charge Nurse.
  • Reports observations and/or symptoms indicative of patient changes and adverse reactions to treatments to the Charge Nurse.
  • Reports care for assigned patients.
  • Helps to set priorities when delivering nursing care.
  • Maintains knowledge of location for needed equipment and supplies.
  • Re-stocks supplies as needed.
  • Reports equipment needing repairs.
  • Cleans and stores equipment when no longer needed.
  • Performs and maintains documentation, including accurate and ongoing assessments, nursing interventions, patient responses, teaching, and discharge status in the care tracker.
  • Completes shift end check prior to leaving duty.
  • Communicates effectively following approved shift report format.
  • Identifies and recognizes abnormal symptoms/changes in patient's condition, established priorities, and takes appropriate action.
  • Communicates and collaborates effectively with other health team members regarding patient condition, satisfaction needs, and recommendations.
  • Promotes and ensures patient and staff safety in all aspects of responsibilities.
  • Utilizes proper body mechanics, transfer/lifting techniques to minimize injury.
  • Implements infectious disease controls, including isolation procedures and proper use of hand washing and gloves.
  • Performs other duties as assigned.
