About The Position

Make a meaningful difference every day as a Certified Nurse Aide (CNA) in our PACE Day Center, providing compassionate, hands-on care and support to frail elderly participants. Working closely with the healthcare team, you will help carry out individualized care plans that promote comfort, dignity, independence, and overall well-being in a warm, participant-centered environment. Under the guidance of the Day Center Supervisor, this role offers the opportunity to build meaningful connections while making a lasting impact in the lives of older adults.

Requirements

• Certified as a Certified Nurse Aide in the State of Michigan.
• High School Diploma or GED required.
• Ability to lift and move objects weighing up to 35 pounds without assistance.
• Reliable transportation.
• Flexibility to provide coverage in the evening and on weekends.
• One (1) year experience working with the frail elderly population.
• Valid driver's license.

Nice To Haves

• CPR certification is desired or obtained within 3 months of employment

Responsibilities

• Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting while following the participant's Life Plan.
• Help participants enjoy their meals and stay hydrated for good health.
• Support mobility, including turning, positioning, and safe transfers.
• Measure and record vital signs, weight, and intake/output.
• Respond with kindness and compassion.
• Report any changes in conditions to the interdisciplinary team (IDT) and leadership ASAP.
• Maintain confidentiality while delivering top-notch care.
