Responsibilities

• Provide personal care and support to patients in their homes and care facilities.
• Assist patients with bathing, grooming, dressing, repositioning, mobility, and other activities of daily living.
• Help each patient feel comfortable, respected, and cared for.
• Work closely with nurses and the hospice team.
• Communicate changes or concerns to the hospice team.
• Help provide the best possible experience for patients and their families.
