About The Position
We are hiring for CMA, CNA, or LPN's for a role that involves onboarding patients into our VCM (Virtual Care Management) program and performing patient monitoring activities. While this role is primarily on-site, remote work and work-from-home opportunities are available. Requirements
- CMA, CNA, or LPN Certification
- Medical Assisting and Patient Care skills
- Knowledge of Medical Terminology
- Experience in Medical Office procedures
- Experience in taking vital sign and weight measures
- Proficiency in managing Medical Records
- Excellent communication and interpersonal skills
- Ability to work independently and in a team
- Must supply your own laptop, home Wi-Fi, and vehicle for commuting
Nice To Haves
- Experience with virtual care technologies is a plus
Responsibilities
- Onboarding patients into our VCM (Virtual Care Management) program
- Performing patient monitoring activities
- Conduct scheduled virtual check-ins with patients (weekly or monthly) to follow up on symptoms and medications
- Respond to software alerts when vital sign anomalies are detected (e.g., dangerously high blood pressure)
- Be present at the doctor's office at least 3 days/week to enroll patients in the RPM program
- Introduce the program and walk patients through enrollment
- Continue remote monitoring from a dedicated contractor station during downtime between enrollments
