The role involves onboarding patients into our VCM (Virtual Care Management) program, and performing patient monitoring activities. While this role is primarily on-site, remote work and work-from-home opportunities are available. Requirements

• CMA, CCMA or LPN Certification / License
• Medical Assisting and Patient Care skills
• Knowledge of Medical Terminology
• Experience in Medical Office procedures
• Experience in taking vital sign and weight measures
• Proficiency in managing Medical Records
• Excellent communication and interpersonal skills
• Ability to work independently and in a team

Nice To Haves

• Experience with virtual care technologies is a plus

Responsibilities

• Onboarding patients into our VCM (Virtual Care Management) program
• Performing patient monitoring activities
• Conduct scheduled virtual check-ins with patients (weekly or monthly) to follow up on symptoms and medications
• Respond to software alerts when vital sign anomalies are detected (e.g., dangerously high blood pressure)
• Be present at the doctor's office at least 3 days/week to enroll patients in the RPM program
• Introduce the program and walk patients through enrollment
• Continue remote monitoring from a dedicated contractor station during downtime between enrollments
