About The Position
We are seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our healthcare team. The ideal candidate will provide high-quality personal care to patients in their home under the supervision of a Registered Nurse. This role requires a strong understanding of safety protocols, effective communication skills, and the ability to work collaboratively with healthcare professionals to ensure the patient's comfort and dignity.
Requirements
- Current CHHA license/certification in good standing.
- CPR Certification (from American Heart - preferred)
- Strong interpersonal skills with the ability to build rapport with patients and families.
- Ability to follow a written plan of care and adhere to safety guidelines.
Nice To Haves
- Familiarity with electronic visit verification (EVV) or electronic health records is preferred.
Responsibilities
- Provide personal care and assistance with Activities of Daily Living (ADLs) including bathing, grooming, dressing, and toileting as ordered on the plan of care.
- Monitor patient status and report any changes in physical or mental condition to the Registered Nurse in a timely fashion.
- Assist with safe transfers, ambulation, and range of motion exercises.
- Maintain accurate daily visit records and document care provided electronically or on paper.
- Assist with light housekeeping, meal preparation, and medication reminders.
