About The Position

The Certified Home Health Aide (CHHA) role involves providing direct patient care in accordance with the plan of care established by the case manager. This position requires maintaining open communication with the case management team, ensuring patient safety and well-being, and performing assigned tasks with competency. The CHHA will also be responsible for accurate and timely documentation, providing emotional support to patients and their families, and adhering to all regulatory requirements. The role emphasizes service excellence and compassionate care.

Requirements

  • Possess and maintain certification as a Home Health Aide or equivalent as approved by the Agency.
  • Satisfactory completion of Skill Assessment Test.
  • Able to record messages and keep records.

Nice To Haves

  • One year satisfactory employment history preferred.
  • Experience in care of the elderly preferred.

Responsibilities

  • Maintain open communication with case manager and/or Team Leader/Assistant Team Leader regarding patient's health, safety, and environment status.
  • Report all changes in patient status in a timely manner.
  • Comply with State/Federal regulatory acts.
  • Perform all skills with competency, as indicated on the skills checklist.
  • Communicate with Clinical Coordinator regarding scheduling concerns, including changes in times, days, patient absences, and discharges.
  • Communicate with case manager, or Team Leader/Assistant Team Leader if case manager is unavailable, regarding case concerns, patient problems, planned time off, or general schedule changes.
  • Complete and submit required documentation, such as patient and employee time sheets, accurately and on time.
  • Report all concerns of possible patient abuse or neglect to the case manager and/or Team Leader.
  • Establish and maintain a therapeutic relationship with patients.
  • Perform only those tasks prescribed by the case manager on the home health aide plan of care.
  • Encourage patient independence by promoting participation in personal care, and in interests and activities within the home, family, and community.
  • Provide or assist with post mortem care after pronouncement.
  • Perform additional tasks as assigned by the Team Leader and/or case manager, depending on the situation, Home Health Aide’s experience, and capabilities (e.g., routine uncomplicated colostomy care and decubitus care).
  • Provide understanding and emotional support to patient and significant others.
  • Participate in the Patient Transportation program for the Private Services department, if eligible.
  • Perform all other duties as assigned.
