This position involves assessing patient and family needs, reporting findings to the RN and MD, and following the prescribed plan of care. The LPN will implement the medical plan of care and nursing interventions, evaluate patient responses, and identify ancillary services needed. Collaboration with the Team Leader, Office Nurse, RN, and inter-disciplinary team is essential. The role also includes assisting patients and families in accessing community resources, maintaining clear and concise documentation, and providing service excellence in accordance with VNA's CARES principles.
