Certified Home Health Aide, Home Health (per diem)
About The Position
Assisted is hiring a per diem Certified Home Health Aide for its Culver City Home Health branch, with about 10 visits per week. The role provides personal care and support to patients according to their needs and the written CHHA Assignment and plan of care. The aide communicates regularly with the RN, Rehab Case Manager, or Clinical Supervisor about changes in a patient’s condition or needs. Candidates must have a current, active California CHHA certificate issued through the California Department of Public Health, along with the other listed qualifications. Requirements
- Complete an approved training program pursuant to Federal/State law.
- Hold a current, active California CHHA certificate through the California Department of Public Health.
- Have a criminal background check.
- Maintain current CPR.
- Have health clearance.
- Complete a skills assessment.
Responsibilities
- Assist patients with personal hygiene, elimination, and nutritional support, as well as ambulation and exercise, according to patient needs and the CHHA Assignment.
- Ensure quality and safe delivery of CHHA services according to the written plan of care.
- Communicate regularly with the RN, Rehab Case Manager, or Clinical Supervisor regarding changes in a patient’s condition or needs.
