The Certified Home Health Aide (CHHA) will be responsible for providing or assisting with ADL activities, including personal hygiene, grooming, and care of mouth, skin, and nails. They will assist with toileting, positioning, and transfer activities, including frequent turning and repositioning of bed-bound residents/patients. The CHHA will also assist with prescribed exercises and Range of Motion (ROM) activities as taught by the Hospice Nurse, and perform light housekeeping duties in the resident's personal unit, such as changing bed linens and doing patient laundry. Additionally, they will feed and assist residents/patients with meals, obtain vital signs (temperature, pulse, respirations) under the supervision of the Hospice Nurse, and report any changes in the resident's/patient's condition to the Hospice Nurse and/or facility nurse, documenting the report. The CHHA will also assist or remind patients to take medication in compliance with regulatory requirements.
