Requirements
- NJ Certified Home Health Aide (CHHA) License in good standing.
- Ability to read, write, and fluently communicate in English.
- Capable of lifting, pushing, and pulling up to 50 lbs.
- Willingness to travel within your assigned territory.
Responsibilities
- Provide personal care, including assisting patients with bathing, grooming, dressing, skincare, and mobility support.
- Prepare nutritious meals tailored to individual dietary needs.
- Maintain clean, safe, and comfortable patient living spaces.
- Use Celltrak to accurately record patient care activities and ensure smooth communication.
- Collaborate with RN Case Managers to monitor and report changes in patient conditions.
