About The Position
This position provides high quality, compassionate healthcare promoting health and wellness in our community. The CDCES works directly with patients and their families to provide individual and group education and support to people with diabetes and other chronic health conditions.
Requirements
- RN, Dietitian – BS, Social Work – MSW, or Exercise Physiologist -MS
- Two years experience in nursing, dietetics, social work, or exercise physiology, including patients with diabetes.
- Certified Diabetes Care and Education Specialist
- Licensure required by State of Alabama dependent on professional designation.
- BLS within 90 days of DOH/Transfer
Nice To Haves
- Advanced degree
- Two years experience in diabetes patient care management and education.
