About The Position

Provides diabetes self-management education/training (DSMT) and/or medical nutrition therapy (MNT) to individuals, family members, and groups. Manages the care of a distinct patient population through assessment of educational needs, nutritional guidelines, lifestyle interventional goals, and counseling. Coordinates patient care in collaboration with other healthcare providers (physicians, pharmacists, case managers, social workers, etc.) to develop effective individualized protocols (medication, therapy, nutrition, devices, etc.) that empower patients with techniques and behavior modifications to help control their disease, avoid potential complications, and bring about lasting improvements in their quality of life. Provides specialized care for Gestational Diabetes Mellitus.

Requirements

  • Bachelor's degree in Dietetics, nutrition, or related field
  • Registered Dietitian (RD)
  • Certified Diabetes Care and Education Specialist (CDCES/CDE) (for employees hired prior to 05/03/2024: within 2 years)
  • BLS-Basic Life Support Healthcare Provider (not required for fully remote incumbents)
  • 2 years recent relevant experience
  • Proven competency in standard concepts, practices, and procedures within the diabetes education specialty field.
  • Demonstrated expertise in the use of diabetes monitoring and managing equipment, including blood glucose monitors, insulin injection devices, and the computer software associated with these devices.
  • Advanced knowledge of adult learning theory and ability to provide individual and group education.
  • Exceptional attention to detail with time management and organization skills.
  • Effective decision-making and delegation skills.
  • Solid knowledge of computer applications, such as Microsoft Office Suite and EPIC to enter/update electronic medical records (EMR).
  • Ability to prioritize activities and work within defined policies, procedures, and practices to achieve objectives and meet deadlines.
  • Ability to use experience and existing precedents or procedures to resolve atypical or infrequent problems.
  • Ability to work independently as well as be part of the team.
  • Ability to accomplish multiple tasks in an environment with interruptions.
  • Ability to communicate technical or sensitive information clearly and professionally to diverse audiences (patients, patients’ family/support group, physicians, and staff).
  • Ability to make independent judgments while caring for patients.
  • Ability to make treatment decisions within their scope of practice and make recommendations to physicians/APCs for medication adjustments.
  • Ability to work autonomously and carry a panel of patients they regularly follow.
  • Ability to be responsible for patient care while working and maintaining that ongoing care day to day and week to week.
  • Ability to collaborate and extend care beyond the provider visit, including follow-up or between provider visits.
