About The Position

Community health worker – certified role will collaborate with healthcare providers in assisting patients with education and support to improve healthcare outcomes. They provide intensive case management to patients and assist with access to care, barriers in obtaining necessary resources; both community and insurance based. In addition, they will act as a guide in navigating transitions of care during a patient's health care journey.

Requirements

  • High School Equivalent / GED (Required)
  • Knowledge of resources and programs available in the Greater Cleveland area (Required proficiency)
  • Bilingual (preferably Spanish) (Required proficiency)
  • Computer competency (Required proficiency)
  • Multi-tasks and prioritizes work (Required proficiency)
  • Teaching skills (Required proficiency)
  • Communicates effectively with persons of various backgrounds (oral and written) (Required proficiency)
  • Maintains a calm, professional demeanor when dealing with internal and external contacts (Required proficiency)
  • Community Health Worker (CHW) (Required)

Nice To Haves

  • Experience in community and population health management (Preferred)

Responsibilities

  • Serves as liaison between service lines, health populations and patient/family in community setting.
  • Provides advocacy and support as needed to remove barriers, preventing patients and families from receiving services or entitlements as needed.
  • Provides basic health literacy and information about how to best access and utilize the health and social service system.
  • Provides assistance to patients and families in meeting needs to include safe and adequate housing, day care, food, household goods and appliances, food assistance and other benefits including health insurance, prescriptions and medical equipment.
  • Coordinate logistics to support care plan goals and interventions – reminders, transportation, and other needed arrangements.
  • Communicates with insurance case managers to advocate for needed services.
  • Builds the capacity of clients/families to address health and social issues.
  • Continuously expands knowledge and understanding of community resources, services and programs provided.
  • Works with other CHWs and staff to update and maintain a directory of community resources (e.g. food banks, housing assistance programs, childcare resources, etc.) in Greater Cleveland area.
  • Maintains reports, provide necessary documents and data by deadlines and upon request of supervision.
  • Has basic knowledge of Ohio Medicaid plans and ability to link clients with case management services.
  • Works collaboratively with clients to link them with resources at UH and within the community.
  • Works collaboratively with the healthcare team to improve outcomes for patients and families in the community.
  • Interprets, translates, or provides cultural mediation related to health services or information for community members.
  • Advocates for individual or community health needs with government agencies or health service providers.
  • Performs other duties as assigned.
  • Complies with all policies and standards.

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
