About The Position

In partnership with the health services team, the Memory Care Caregiver is responsible for assisting residents in Memory Care with all aspects of care, engagement, and dining as assigned by the Enliven Director or designee. This is a full-time position working the NOC shift, Tuesday through Saturday.

Requirements

  • Arizona Caregiver Certification required
  • High school diploma or GED
  • First Aid / CPR Certification required

Nice To Haves

  • Experience with Yardi, eMAR, Schedule Pop, ADP, or other relevant platforms.

Responsibilities

  • Adheres to and conveys the SRG philosophy of person-centered care and Genuine Hospitality.
  • Maintains a professional appearance, manner, and approach while on duty.
  • Wears name tag, uniform and adheres to SRG cell phone policy.
  • Reports to work on time and as scheduled per community policies and procedures.
  • Receives report from the previous shift, reads Care Staff Assignment and End of Shift report for information and changes before providing resident care.
  • Completes incident reports when needed.
  • Signs out and carries equipment such as radio, tablet, and keys at the onset of shift.
  • Documents care given via CareStream; adhering to policies and procedures.
  • Provides report to oncoming shift and documents on the End of Shift Report and CareStream before going off shift.
  • Answers resident call lights, door, and resident alarms promptly.
  • Provides resident assistance with activities of daily living, encouraging and promoting residents’ self-care to the extent possible as needed or assigned.
  • Assists residents with meals, housekeeping, and laundry services as needed, requested or assigned.
  • Sets up the dining room, serves food, and cleans up the dining and kitchen area.
  • Conducts engagement as assigned, encourages and assists residents in individual and group program activities.
  • Maintains a clean, neat, comfortable, safe environment for residents, staff, and visitors.
  • Provides high-quality resident care and recommends ways to improve quality.
  • Observes for changes in resident physical or mental status and communicates to the Enliven Director, LVN/LPN, and/or manager on duty.
  • Responds promptly to residents, staff, or visitor emergencies, follows policies and procedures.
  • Demonstrates knowledge of and follows policies and procedures for infection control.
  • Familiar with fire, missing person, and other emergency procedures; follows community policy and procedures detailed in the fire and disaster plans.
  • Maintains appropriate levels of confidentiality with resident and family information.
  • Attends and participates in training and team meetings as assigned.
  • Other duties as assigned.
