Join our Adult Intensive Services Division as a Certifications Case Manager and play a vital role in supporting adults with serious and persistent mental illness on their path to stability and independence. In this dynamic position, you’ll partner closely with our post-hospital and Hospital Liaison teams to coordinate care for clients certified for outpatient treatment, ensuring they remain engaged in services and compliant with court-ordered processes. You’ll serve as a critical bridge between clients, clinical teams, and the legal system—delivering and filing certification paperwork, supporting court petitions, managing documentation, and maintaining accurate tracking systems. This role offers a meaningful mix of office, remote, and community-based work, ideal for someone who is highly organized, detail-oriented, and passionate about client recovery.
