About The Position

Temple University Hospital's Episcopal Campus is seeking a Certified Diabetes Care and Education Specialist (RN) to deliver comprehensive educational support services to patients and their families/care partners. This role aims to bridge the gap and integrate clinical and self-management aspects of diabetes, providing collaborative, person-centered care conducive to behavior change and improved quality of life across the lifespan. The specialist will support and advocate for those affected by diabetes to optimize care, promote self-management to achieve individualized goals, and reduce health risks. Responsibilities include participating in case management, patient care planning, goal setting, and developing/maintaining education curricula and materials. The role also involves assisting the Education Coordinator in assessing, planning, implementing, and evaluating the Patient Centered Diabetes Education program, and collaborating with other educators to identify needs and develop clinical programs. Additionally, the specialist will promote and disseminate diabetes information to professionals and the public, following American Diabetes Association guidelines.

Requirements

  • Bachelor's Degree Nursing Required
  • 3 years experience as a Registered Nurse Required
  • 3 years experience in Diabetes self-management education and support experience Required
  • General Experience and working knowledge of diabetes technology Required
  • General Experience working with a diverse patient population Required
  • PA Registered Nurse License Required or Multi State Compact RN License Required
  • Cert Diabetes Care and Ed Spec Within 1 year Required

Nice To Haves

  • 1 year experience in a community health or ambulatory care setting Preferred
