About The Position

We partner with health systems, community health centers and independent practices to remove barriers to care to ensure all people have access to the care they deserve. We are looking for Central Patient Engagement Specialists to play an essential role at AndHealth. This position is located at our corporate office in Columbus, OH.

Requirements

  • Registered or Certified Pharmacy Technician in the state of OH required.
  • Must be willing to obtain and maintain licenses or certifications in multiple states if applicable.
  • Tuberculosis (TB) Screening: Documentation of a negative screening conducted within the last 12 months.
  • Hepatitis B: Proof of a completed vaccine series or positive blood titers from within the last 12 months.
  • Measles, Mumps, & Rubella (MMR): Proof of a completed vaccine series or positive blood titers from within the last 12 months.
  • Varicella (Chickenpox): Proof of vaccination, documented medical history of the disease, or positive blood titers from within the last 12 months.
  • Tdap (Tetanus, Diphtheria, Pertussis): Documentation of a booster administered within the last 10 years.
  • Influenza: Proof of vaccination for the current flu season.
  • COVID-19: Documentation of primary vaccine series and any boosters administered.
  • General: Additional health requirements may be requested as necessary.

Nice To Haves

  • A bachelor’s degree is preferred.
  • Basic Life Support (BLS) obtained or willing to obtain.
  • Nationally certified - PTCB or ExCPT is preferred or willing to obtain post hire.
  • 3+ years of pharmacy experience is preferred.
  • Excellent written and oral communication skills.
  • Strong attention to detail and ability to handle multiple things simultaneously.
  • Comfortable with technology and the ability to adapt to new systems.
  • Must be organized and able to work independently.

Responsibilities

  • Help patients in disease management programs obtain and maintain access to specialty medications.
  • Complete, submit, and track prior authorizations and appeals based on clinical and insurance criteria.
  • Proactively renew prior authorizations ahead of expiration to prevent gaps in therapy.
  • Monitor patient adherence through fill verification and outreach to patients and pharmacies.
  • Coordinate with pharmacies on prescription processing, fills, and deliveries.
  • Connect patients with financial assistance programs and affordability resources.
  • Notify providers of formulary restrictions and collaborate on medication adjustments.
  • Escalate through phone and urgent channels when medication needs are time-sensitive.
  • Route all clinical interventions or counseling needs to an appropriate licensed clinician.
  • Collect and organize medical records and prescription documentation.
  • Perform data entry and administrative support for pharmacy operations.
  • Conduct outreach to encourage utilization of community health center pharmacy services.
  • Conduct adherence checks and provide general operational support to pharmacy sites.
  • Coordinate prescription transfers into partner pharmacies.
  • Serve as a floater technician across pharmacy sites as needed.
  • Generate reports on new patients, upcoming medication needs, and adherence data.
  • Conduct outreach to recruit eligible patients with complex, polychronic conditions into the program.
  • Serve as a patient navigator, coordinating care across multiple providers and services.
  • Coordinate with outside providers to ensure continuity of care.
  • Facilitate prescription transfers for centralized pharmacy management.
