About The Position

Responsible for supervising Activity Assistants/CNA’s (Certified Nursing Assistants), conducts routine admissions of clients, and interacts with caregivers. Ensure the policies and procedures of the organization and any licensing or contracted agencies are enforced.

Requirements

  • Graduated from an accredited four (4) year college or university.
  • No less than one (1) year experience in working with people in human services or medical related programs.
  • Excellent office administration skills with advanced knowledge of Microsoft Office programs.
  • Must be able to use reasoning abilities to manage multiple tasks simultaneously and prioritize duties and assignments effectively.
  • Must be able to relate in a positive and warm manner to the elderly and to understand the issues of confidentiality, sensitivity to the needs of those living with dementia and their families and caregivers.
  • Must have reliable, personal vehicle and a valid Driver’s License, good driving record, and applicable insurance coverage required by the company policy.

  • Have mobility to work in a standard office setting and to use standard office equipment, including a computer; vision to read printed materials and a computer screen; and hearing and speech to communicate in person and over the telephone.

  • Ability to work in a highly stressful environment dealing with a wide variety of challenges, deadlines, and diverse employee population.
  • Able to move about freely while assisting clients with tasks, activities, and movement.
  • Able to assist clients with transfers from sitting to standing and assist in walking.
  • Able to occasionally lift up-to twenty-five (25) pounds to move office supplies or other equipment.

Nice To Haves

  • Experience working in nursing homes or other senior care facilities is preferred.
  • Experience working with the elderly or individuals living with Alzheimer’s or dementia preferred.
  • Bilingual (English/Spanish) preferred, and ability to read and write in Spanish strongly preferred.

Responsibilities

  • Manage the daily operations of Grace Place Alzheimer’s Activity Center.
  • Supervise, train, and evaluate the performance of Activity Assistants/CNAs, ensuring staff adhere to safety regulations and maintain compliance with licensing requirements.
  • Work with Human Resources and Director to ensure appropriate onboarding, training, monitoring and evaluations of Activity Assistants/CNAs.
  • Ensure the engaging activities are planned and scheduled for clients and maintain and support individual client care plans.
  • Work with Lead CNA to develop and post Monthly Activity Calendar and make it available for caregivers.
  • Supervise staff to ensure that all client activity participation and attendance records are accurate and monthly progress notes are signed, dated, and complete.
  • Maintain a center census of clients, assist in handling requests for information from the community, provide tours, manage referrals, and participate in outreach as needed.
  • Maintain accurate client data with existing databases such as Excel spreadsheets, Word documents, etc.
  • Support Director and nursing staff to ensure data is current and accurate.
  • Support new client onboarding and admissions including client screenings recruitment, admissions, and decisions to disenroll.
  • Work with the finance team to submit client billing and program fee information.
  • Provide for safety of clients through appropriate behavioral management and intervention.
  • Work with Director to manage compliance and renewal of state and local licensure requirements for an adult day center.
  • Plan and implement staff development with goals of continuous program improvement, staff and client safety and compliance with regulations regarding mandatory training and in-service.
  • Attend required in-service training to continually maintain and improve the quality of the center’s daily operations.
  • Assist with meal delivery when needed.
  • Other duties as assigned by management.
