Broad Street, Philadelphia, PA 19147, has an opportunity for an exceptional Center Counselor who is versed in the physical, mental, and emotional aspects of older clients. The Center Counselor is responsible for client contact, assessment and follow up to promote resolution of problems and to encourage development of the client’s potential to deal with his/her environment. Requirements

Education: Associate’s degree required.
Knowledge of the physical, social and cultural aspects of the community by the agency; knowledge of the techniques of interpersonal relations, personal assistance and community organizations; ability to relate to clients and provide positive support.
Ability to perform work with limited supervision.
Certificate/Licenses:

Work Experience: One to two years social work experience working with seniors preferred .

Responsibilities

Telephones contacts to clients for need determination, information referral and advocacy sufficient to resolve problems.
Requests for all home visits must be approved by Center Director.
Demonstrates adequate use of referral resources to meet client needs.
Participates in routine outreach assistance.
Checks progress and provides additional support by consistent follow up.
Records relevant case information in individual client case record.
Encourages client participation in Agency programs when appropriate and coordinates, through the Center Director, and program departments.
Links up interested individuals with existing SWE groups.
Supervises the assignment of new cases and also assignments to aides.
Ensures that follow up contacts are made monthly with new clients and every four months for active clients.
Responds to complex problems that affect the client experience.
Adheres to the Social Service Standard Operating Procedures.
Conduct Group Counseling sessions.
Performs additional responsibilities that may be assigned as necessary.
Must work the days and hours necessary to perform all assigned responsibilities and tasks. Must be available (especially during regular business hours or shifts) to communicate with supervisors, customers, vendors and any other person or organization with whom interaction is required to accomplish work and employer goals.
Must develop and maintain courteous and effective working relationships with clients, vendors and/or any other representatives of external organizations.
Must assume responsibility for providing timely, appropriate responses to requests/suggestions/complaints or refer such comments to the appropriate person. When referring requests/suggestions/complaints to another individual, the employee must follow-up in writing to the individual and must maintain an accessible copy of the referral, or follow company guidelines.
Must communicate information and state problems or challenges to be resolved in a clear, concise, courteous, nondiscriminatory and professional manner, and be able to provide clarification, as necessary.
Must show respect for the opinions of others and behave in such a way as to ensure an atmosphere free of needless interruptions, difficulty and/or discrimination.
Must receive and provide clear, constructive feedback regarding work performance (to/from individuals or groups) and be able to provide clarification, as necessary.
